• Download from here and fill in the Request for recommendation to obtain a Provisional Residence Permit (MVV)
• Collect the needed documents:
  1. The Sponsor (Guarantor) Declaration Annex
  2. The fully completed and signed Employers Declaration Annex(s)
  3. An extract from the Municipal Administration showing your address in The Netherlands and family composition
  4. Copies of the front and back of your residence permit
  5. A translated legalized copy of unmarried status declaration of the inviter
  6. A translated legalized copy of unmarried status declaration of the foreign nationals (partner)
  7. Copies of all of the pages of the foreign nationals passport
  8. A copy of the current employment contract
  9. Documentary evidence that the employer has registered the current employment contract for the foreign national with the competent authority in connection with the payment of social security contributions and taxes. This can be the companies' registration certificate.
  10. Payslips for the last 3 months
• Send documents to IND (Immigratie- en Naturalisatiedienst), t.a.v. afdeling MVV, Postbus 70567, 5201 VZ, `s-Hertogenbosh. The documents can be processed at longest of 3 months. Call IND if the procedure takes too long time.
• In case of positive reply from IND (usually it states “I see no reasons to disallow the person to enter Netherlands”), you need to pay €830 before IND will continue with application. During payment to need to mention V-number and dossier number in comments for the transfer.
• The payment is processed within maximum 1 week and confirmation is send to the embassy of the country of origin of your partner. The embassy sends a letter to the partner advising to make an appointment for making MVV visa. The following documents needs to be taken to the embassy (may vary depending on the country; the list should be mentioned in a letter from embassy):
  1. Usual application for for Schengen visa
  2. Passport photo for Schengen visa
  3. Original unmarried status declaration of the inviter
  4. Original unmarried status declaration of the partner
  5. The copy of ID of the inviter
  6. The copy of passport of the inviter
  7. Birth certificate
  8. Partner's ID (and travel passport, if applicable)
• After MVV visa is issued, the partner has to enter Netherlands within 6 month from the date of issue. In a meanwhile the partner receives the letter from IND with the instructions of what to do next together with necessary forms to be filled in before the appointment.
• After arrival, the partner has to be registered at Municipal Administration at the same address as inviter. If partner is already registered but at different address (e.g. stayed previously for the work or study) you don't need to request a certificate of the registration (see below), otherwise ask for an extract. Also ask about the dutch courses for foreigners.
• Visit GGD and make tuberculosis test (take the corresponding form send by IND).
• Make an appointment for IND to submit the documents for permanent visa (request for permanent stay with a parter having MVV visa). Take the following documents with you:
  1. Partner's travel passport with MVV
  2. The copy of inviter's ID
  3. An extract from Municipal Administration which states that partners leave together (see above). If the partner was already registered in Netherlands for different address, you need only the inviter's extract, as partner will be re-registered at IND at the same day (at least that what can be done at IND located at Stadhoudersplantsoen 26, 2517 JL, Den Haag, NL).
  4. The non-criminal statement form (empty formular is send by IND)
  5. Relation explanation form (empty formular is send by IND)
  6. New passport photo and signature should be provided on a separate form (empty formular is send by IND)
  7. Tuberculosis test results (empty formular is send by IND). If you wish to do it after this visit to IND, the result of the test should be mailed to IND.
     Put signatures (date and place if needed) on:
  • Request form, title page
  • Appendix 2 for IND, last page
  • Relation explanation form (also fill in the date a partner started to leave with the inviter)
• Register at tax office and receive a tax-number
